Current status and plan for patient video consultations and SMS services

As outlined in 6 November bulletin, many practices have been using AccuRX, MJog and similar products to communicate extensively with patients, especially since March.

Whilst awaiting a new framework which is due to launch in January, the digital transformation team are currently undertaking an evaluation of options in the market as there are many new offerings. Nationally, there is not clarity on funding for these services; however, central funding is normally linked to buying from frameworks, so once this is known, the team can understand what costs need to be picked up at CCG and practice level. Currently practices pick up costs for SMS solutions and the CCG picks up the SMS charges from BT/EE. Video has always been picked up through central funds.

A clinical evaluation group has formed (please advise if you would like someone in your PCN to attend future meetings) to review insights from GPs and patient feedback and outline key features to support the pathway. The digital transformation team will share this information shortly. The CCG has had a number of conversations with AccuRX and MJog, who are keen to sign deals before Christmas on a CCG level.

Currently AccuRX have a free product which will stay “free forever”. During the COVID response, video and advanced SMS (two way messaging, surveys and attaching documents) have been covered by national funding. From April (possibly June), the video and enhanced SMS options are going to be chargeable locally. New features are to be launched, including a triage form, batch messaging, appointment reminders and additional online questionnaires. Costs have been quoted at 59p per patient (average of £3,794 per practice) for the full bundle.  

There has also been a competitive offer from MJOG, who are offering something equivalent but less developed compared to AccuRX at a fraction of the cost (£403 per practice). They have just been bought up by Livi and have big plans that are still emerging.

Despite these offers the digital transformation team would still recommend waiting for the framework and, where possible, it would be beneficial to move forward as one county. It is believed that it would provide the best value for money and a clearer view on the market (for example, the team has been told that TPP are planning to match the AccuRX functionality within the current costs of the clinical system). IT Access Rights for General Practice

Practices that have and are transitioning to the single domain will be connected to a shared countywide network that is managed by Countywide IT Services (CITs).  With all PCs connected to the network, enables CITs to centrally manage software updates, patches and react to cyber-attacks on the network.  To manage the cyber risks, CITs are applying enterprise level cyber security measures to protect clinical systems and patient data from potential cyber-attacks. 

Prior to practices migrating to the single domain, practice managers and selected surgery staff were able to conduct a number of IT administrative tasks.  As part of the migration to the single domain, IT administrative tasks are handed over to CITs and designated representatives of the service to provide ongoing management and support of IT tasks.

Designated practice staff will continue to manage NHS mail accounts for their respective practices as Local Helpdesk.

Local helpdesk administrators will be able to reset, unlock and amend accounts

All other IT administrative functions such as new starter accounts or leavers etc will require surgeries to contact or log requests with the CITs Service Desk.
